Empowerment Through Plastic Surgery, with Dr. David Pincus, MD, FACS

As we reach the end of the year, many people will make New Year’s resolutions to lose weight, eat healthier, and empower themselves. Many will keep their resolutions. Others will watch them go by the wayside. But one industry specializes in empowering you all year long. Dr. David Pincus, of Pincus Plastic Surgery in Smithtown and Boca Raton, Florida sees a steady stream of patients who want to use cosmetic surgery to look, or feel, better.

From fillers to Facelifts, Liposuction to lip surgery, Americans are adopting plastic surgery as part of their lives. “More people are turning to plastic surgeons to change their appearance,” Dr. Pincus said “There are a lot of options.” Dr. Pincus obtained his medical degree from the University of Miami School of Medicine and served his general surgery residency at Jackson Memorial Hospital in Miami, a plastic surgery residency at Lahey Hospital, and Medical Center in Burlington, Mass. During his plastic surgery residency, he also trained at Brigham and Women’s Hospital, Boston Children’s Hospital, Maine Medical Center, and Emerson Hospital in Massachusetts. Dr. Pincus’ awards include being named among the 10 best plastic surgeons in 2018 by the American Institute of Plastic Surgeons.

As masks came off as the pandemic waned, plastic surgery boomed. More specifically, there has been a 19% increase in cosmetic surgery procedures since 2019. Breast procedures grew by 15% and face procedures grew by 18%. “Throughout the course of the past several years, plastic surgery itself experienced a transformation,” ASPS President Gregory A. Greco wrote in a 2022 report. “Not just in terms of techniques and technology, but how it is perceived. Patients have become more transparent and comfortable in sharing their journeys.” If plastic surgery traditionally had a stigma, it has become not just more popular, but less of a secret and more of a subject of conversation. “The pandemic served as a period of reflection, steering many to prioritize personal comfort and well-being,” according to the ASPS report.

POST-PREGNANCY PLASTIC SURGERY

While the vast majority of procedures are done for women, pregnancy itself has been spurring a new transformation as well. The effects of pregnancy on a woman’s body can remain long after the glow of childbirth, body, such as pockets of fat, loose abdominal muscles, and sagging breasts,

can remain long after the glow of childbirth. Plastic surgery can provide a slimmer waist, fuller breasts, and other changes in a “Mommy Makeover,” in which a plastic surgeon uses two or more cosmetic surgery techniques to target changes to a woman’s body from pregnancy. “Plastic surgery can be a tool to help address aging, but it also can help undo undesirable effects of pregnancy,” Dr. Pincus said. “A Mommy Makeover can reshape the body and restore a woman’s pre-pregnancy figure.” During pregnancy, glandular breast tissues swell and the skin stretches due to milk production. Tissues can remain enlarged until nursing ends, before breasts lose volume, often becoming smaller or sagging more. “Breast augmentation or a breast lift can restore youthful breasts,” Dr. Pincus said. “After a woman gives birth, there are things that can be done to address changes that occur.” Pregnancy also can affect hormone levels, resulting in pockets of fat that resist diet and exercise. Unwanted fat can accumulate in various parts of the body, decreasing confidence and self-esteem. “Liposuction is a minimally invasive surgery that can remove stubborn fat and recontour almost any area of your body,” Dr. Pincus said. A woman’s uterus expands during pregnancy, putting pressure on the abdominal muscles and often causing them to separate while the waistline widens and skin stretches. “A tummy tuck can tighten loose muscles and remove excess to give you a firmer and more slender midsection,” Dr. Pincus said. A tummy tuck, liposuction, breast reduction, and breast lift results are considered permanent, but they can’t stop the effects of age and gravity, Dr. Pincus added. “You can expect long-lasting results unless you experience significant weight fluctuations or subsequent pregnancies,” he said. “Breast augmentation can last 10 years or more unless a complication arises.”

FACE OF THE FUTURE

Our faces are the canvases on which we paint emotions, but age also appears on the face as skin loses elasticity. More people are turning to surgeons to slow aging on the face: Facelifts grew by 8% from 2021 to 2022 to 72,700, according to the ASPS. “A facelift is a cosmetic surgical procedure that lifts and firms sagging facial tissues to restore a more pleasing facial appearance,” Dr. Pincus said. A facelift (or rhytidectomy) can help improve lines, wrinkles, sagging skin, jowls, nasolabial folds, and facial hollows. Advances in surgical technology can produce a natural look, “highlighting natural beauty and subtle contours,” Dr. Pincus added. Facelifts vary depending on desired outcomes and the extent of aging. Facelift surgery is performed under general anesthesia and takes about two to three hours to complete. “It’s an outpatient procedure, and you can return home to start your recovery within a few hours,” Dr. Pincus said. In a traditional facelift, incisions are made in the hairline at the temples from the lower scalp, extending behind the ear. Excess fat from the neck and chin are trimmed and facial tissues and muscles are tightened before incisions are closed. A limited incision facelift is less invasive, focusing on a mild degree of jowling and sagging skin in the lower face with shorter incisions. “A facelift won’t stop aging,” Dr. Pincus said “But it does an impeccable job of restoring the face to a more youthful appearance.”

BREAST REDUCTION

Breast enlargements are common procedures, but breast reductions are increasing as women seek to be more comfortable in procedures often covered by insurance. Breast reductions increased by 54 percent from 2019 to 2022, while breast lifts increased by 30 percent and liposuctions surged by 23 %, according to the ASPS. “Women with oversized breasts tend to have cosmetic concerns related to their chests,” Dr. Pincus said, “But the main issues leading them toward breast reduction surgery are often related to their physical comfort.” He said the inability to “comfortably participate in many activities” and “persistent, uninvited attention” can increase discomfort. “For many large-chested women, their breasts significantly impair their ability to live an active and confident life,” Dr. Pincus said. “We can help women have the body they want in so many ways and this is one.” Breast reduction removes glandular tissue, fat, and skin to create more compact, balanced breasts. But the surgery changes more than the body: It can change a life. “This allows women to participate in more activities comfortably and have more confidence when doing so,” Dr. Pincus said. “Breast reduction surgery relieves physical discomfort and helps stop the negative attention women with very large breasts often endure.”

MALE BREAST REDUCTION

While women often seek breast reduction, enlarged breast tissue in men and boys or gynecomastia causes discomfort and diminishes self-esteem in men. This often resolves on its own or through diet and exercise, but gynecomastia surgery or male breast reduction can also correct overdeveloped or enlarged breasts. Male breast reduction from 2019 to 222 increased by 17 percent to about 24,500, according to the ASPS. The best candidates for male breast reduction are healthy nonsmokers within 30 to 40 pounds of their ideal body weight typically after puberty. Liposuction can also be used to remove excess fat through small incisions and plastic surgeons can reposition chest tissues “for a more natural male contour,” Dr. Pincus said. The surgery can result in a firmer, more contoured chest, although lifestyle can impact outcomes over time. “To maintain the results, individuals will need to follow a healthy lifestyle,” Dr Pincus said. “Weight fluctuations can affect the appearance of the chest over time.: Profile of a plastic surgeon Dr. David Pincus is an award-winning, board-certified, award plastic surgeon experienced in a wide range of cosmetic procedures such as breast and body procedures.
